Please remember to bring your completed forms with you for your first appointment. Your First Visit
At your first visit, arrive about 15 minutes early to complete registration forms, or download the forms on this page and complete them at home. Once registration is complete, you’ll meet with Dr. Duong. He’ll listen carefully to your concerns and answer all of your questions. You can expect a thorough oral examination, including:
Health history review
Oral cancer assessment
Necessary digital X-rays
Intraoral photos
Head, neck, occlusion examination
Comprehensive exam
Hygiene consult and cleaning (in absence of gum disease)
Complete treatment plan
Dr. Duong will review this information, focusing on your immediate and long term goals. Then he’ll enter a thorough discussion with you regarding the best treatment plan to meet your unique oral health goals.
We believe, the more you know about treatment options, the more prepared you’ll be to make informed decisions. Dr. Duong will review your X-rays and intraoral images, and explain potential solutions. As your personal, professional dental consultant, Dr. Duong will help you determine the best path to improve and maintain your oral health.
